Methodological Evaluation of District Hospitals Systems in Ghana: A Randomized Field Trial for Measuring Clinical Outcomes

Abstract

In Ghana, district hospitals play a critical role in healthcare delivery, yet their performance varies significantly. A randomized field trial was implemented to measure the effectiveness of district hospital systems. Data collection included patient records and standardised clinical assessments conducted at each site. Clinical outcomes showed an improvement in patient recovery rates by 15% (95% CI: 6-24%) following intervention, with a notable reduction in infection rates among treated patients. The randomized field trial demonstrated the efficacy of targeted interventions for improving clinical outcomes in district hospitals. Further research is recommended to generalize findings and inform policy. District hospital managers should prioritise implementation of evidence-based practices identified during the trial, such as enhanced infection control measures and improved training protocols. Treatment effect was estimated with $\text{logit}(p_i)=\beta_0+\beta^\top X_i$, and uncertainty reported using confidence-interval based inference.
