Methodological Evaluation of District Hospital Systems in South Africa: A Randomized Field Trial for Yield Improvement Assessment

Abstract

District hospitals in South Africa play a crucial role in primary healthcare delivery, but their efficiency varies significantly. A comprehensive search strategy was employed to identify relevant studies from databases such as PubMed and Embase. Studies were assessed using predefined inclusion criteria, including randomized field trial designs focusing on South African district hospitals. The analysis revealed a mixed pattern of yield improvement across different districts, with some showing significant gains in patient throughput by 20% over the baseline period. This review underscores the need for standardised evaluation methods and ongoing quality assurance protocols to enhance hospital efficiency systematically. District health authorities should prioritise implementation of evidence-based yield improvement strategies and continuous monitoring through randomized field trials. Treatment effect was estimated with $\text{logit}(p_i)=\beta_0+\beta^\top X_i$, and uncertainty reported using confidence-interval based inference.
