District hospitals in Tanzania often struggle to meet clinical standards due to resource constraints and inadequate management systems. A mixed-method approach will be employed, including quantitative surveys (n=200) and qualitative interviews (n=25), supplemented by process mapping and statistical analysis of operational data. Randomization will ensure equitable allocation to treatment conditions. Initial findings suggest a significant improvement in patient flow from the pre-trial period to post-intervention, with a 15% reduction in average waiting times reported across participating hospitals. The randomized field trial design provides robust evidence for system reliability improvements and can serve as a model for other healthcare systems seeking to enhance efficiency and quality of care. Implementing standardised protocols and continuous monitoring will be crucial for sustaining observed gains in hospital performance.
