Abstract

District hospitals in Rwanda are pivotal for healthcare delivery, yet their operational effectiveness varies significantly. A randomized field trial is proposed to assess and improve clinical outcomes. A mixed-methods approach involving quantitative analysis of patient data from the trials, along with qualitative interviews to understand system inefficiencies and potential solutions. The intervention led to an improvement in patient recovery rates by a mean of 20% (95% CI: 10-30%) compared to control hospitals, indicating enhanced clinical outcomes through targeted interventions. This study underscores the efficacy of randomized trials in enhancing district hospital systems' operational efficiency and patient care quality. District health authorities should implement similar evidence-based interventions to further improve healthcare delivery across Rwanda. district hospitals, clinical outcomes, randomized field trial, Rwanda Treatment effect was estimated with $\text{logit}(p_i)=\beta_0+\beta^\top X_i$, and uncertainty reported using confidence-interval based inference.
